Refuse sign-up without a way to verify and recover

Signing up with a password with no email verification created a dead end. No verification link is sent, the address stays unverified forever, and both access.ts and mcp.ts require a verified address before a shared brain grants anything. A brain shared with such an account silently granted nothing, with no error explaining it. There is also no password reset, so a forgotten password locked the account permanently. Sign-up must be closed until mail can be sent. Gated on EMAIL_FROM + RESEND_API_KEY rather than commented out, so wiring a provider turns it back on.
