Plan limits lived in three places in three forms; 'agents write back — Pro' was drawn in the UI and never checked anywhere; goal changes cleaned the exam only in web, not in the seed script. All money and access rules must be centralized. Violations of this repeated: lib/plans.ts (one source of truth for limits), lib/goal.ts (goal changes must invalidate exams everywhere), lib/review.ts (approval logic one place).
