Own the identity schema; do not depend on interactive CLI tools in deployment

The better-auth CLI creates migration tables fine on a laptop but silently does nothing inside a container — no output, no error, no tables. The first deploy came up with an empty database and no way to sign in. 0000_auth.sql is dumped from a database the CLI had migrated, matching exactly, so the migration runner no longer refuses to start when the tables are absent. Deployment must not depend on an interactive npx tool.
