Route structured output through forced tool calls, not output_config

Structured output now goes through a forced tool call instead of output_config. Both work against the first-party Anthropic API, but a proxy in front of it accepts output_config and silently ignores it — resulting in markdown back and a JSON.parse error on every request. Tool calling passes through everywhere, so the same code runs on either endpoint.
