Self-refresh: unchanged pages are not re-read, changed pages are re-queued

URL sources maintain a fingerprint (lib/page.ts). Maintenance job (worker/maintenance.ts runs every 6 hours) re-reads old sources. If page fingerprint unchanged, don't re-extract. If changed, mark old notes stale and queue new ingest. If page gone (404), mark orphaned. Checked in scripts/check-refresh.ts.

Adoption vs rewrite: source without fingerprint 'adopts' identity

If a URL source has no stored fingerprint (e.g., imported from elsewhere), the first read adopts that URL as its identity rather than treating it as a new source. Prevents duplicates.
