Reranker is not needed; quality issue is in note content, not ranking

`diagnose` can distinguish 'judge saw but did not count' from 'not found'. On RGS API the ratio is 7 to 2, showing the bottleneck is note quality, not ranking. A reranker would not change the outcome.
