brain_add_source guards against SSRF and credential injection

brain_add_source takes documentation URLs or a block of text and queues the same ingest the web upload uses. URLs go through the existing SSRF guard and text through the secret scanner — an agent handed a link by a poisoned page is exactly how a request to 169.254.169.254 gets made, and this is a path with no human looking at the material first.
