url-guard.ts prevents server-side request forgery in brain ingestion

lib/url-guard.ts prevents SSRF attacks because the worker fetches from inside the network — without it, a user could point a source URL at cloud metadata or an internal admin panel and read the result back as notes. The guard is checked when the URL is added and again at fetch time, since DNS can change in between. Redirects are refused for the same reason.
