CSRF token generation requirements

CSRF tokens must be: (1) Unique per user session; (2) Secret; (3) Unpredictable - generated using a cryptographically secure pseudo-random number generator (CSPRNG). Tokens can be generated once per user session or once per request. Per-request tokens are more secure as the time range for exploitation is minimal, but may hinder browser back button functionality. Per-session tokens are stored in the session and used for each subsequent request until session expiration.
